Here is an example of how tourniquet application can be extremely challenging with large wounds. Shout out to Anthony Ceissman for the photos and back story. Be advised, they are graphic in nature.

In these photos you see the damage done when someone falls into a window after slipping. "High and tight" can't be a go to, and wounds in the junctional areas like the armpit can be very difficult to stop. In order for a tourniquet to be effective it needs to be able to stay tight around the extremity. If you can get one on, great. If not, what are your options for serious arterial bleeding?
Gauze, ideally with a hemostatic agent (or whatever you have available) should be firmly applied and packed if possible, with direct pressure. This is serious direct pressure, so get some curls in beforehand.
